I have not made a t-shirt quilt, but I have quilted one. The requirement that I gave the lady is that the t-shirt squares had to be stabilized so there wouldn't be stretching when I was quilting it. Regarding the difference in sizes I have a quilting friend who is doing a t-shirt quilt for someone else so she has the squares on her design wall with sashing strips where required. The smaller ones can be brought up to size by adding sashing strips, or if the front design is really smaller you can use two of them to make up the size of the block you need. There is really no way that you have to put it together, you just need to decide what you want to use for sashing and have the sashing bring the blocks to the right size.
I recomend the T-shirt group on this site. There are photos and alot of ideas to get you going.
